A decade after an explosion ripped through the Deepwater Horizon drilling rig in the Gulf of Mexico, unleashing the worst offshore oil spill in United States history, all the members of a bipartisan commission set up to prevent such disasters from occurring again say many of their recommendations were never taken seriously and a similar disaster is still possible.
